Data & reporting

From the record to the decision

Behind each product is one rule: clinical teams enter data once, as part of their work, and the reports, returns and warnings follow from it. Here is how that looks in three of them.

One entry, three uses

A unit records each admission once. That same record fills the national return, updates the regional view of beds and is ready when the audit comes round, without anyone typing it again.

Read the CAPANET case

Delays show while they can still be fixed

Every step from referral to discharge has a target. LTV Tracker times each step and turns it amber or red as it nears or passes that target, so the team knows which step is keeping a child in hospital.

About Smartmed

We started on a ward.

Smartmed began on the wards of Royal Brompton Hospital. Our founder, James Woods, built clinical systems there side by side with the clinicians who used them, and the first, e-VENT, won an NHS Innovation Award for Healthcare ICT.

We still start every project the same way: with a clinical problem, and time with the team who live with it. Fifteen years on, that has taken our software from one ward to a national programme for NHS England and a research registry shared with Italian sites.

Process

How we work

Four steps, and the last one keeps going.

  1. We spend time with the people involved, map how the work happens today, and look at the systems and data you already have, so new software fits around them.

  2. Clickable prototypes, reviewed by the people who will use them, before we build.

  3. Built in short stages, with working software to test at each one.

  4. Hosting, updates and changes guided by your team, long after go-live.

    Questions

    A few good questions.

There is no need to arrive with a finished specification. A clear description of the problem is a useful first step.

Tell us what you have in mind
Can you turn our spreadsheet into a system?

Yes. A spreadsheet is a good place to start: its columns, forms and reports already show what the team records and what it needs back. We look at the work behind it, then scope a system the whole team can share and report from.

    See CAPANET in practice
Can you help with something we already use?

Often, yes. Tell us what the system does, what still works and where it gets in the way. From there we can say whether it is better to improve it, build around it or replace it.

Who owns the software and data?

The NHS platforms on this site are owned and operated by the organisations that commissioned them. On a new project, who owns the software, who is responsible for the data and how handover works are settled at the start, before anything is built.

Where can the application be hosted?

On the NHS network, among other options. CAPANET and LTV Tracker both run on the NHS Health and Social Care Network, and we advise on the right hosting for each new system.

    What happens after launch?

We stay involved. Maintenance, updates and improvements carry on once the system is in use, guided by the people using it. How much support you need, and how changes are requested, is agreed with you.

    More about ongoing support
What affects the cost and timescale?

Mainly four things: how many people will use it, how complex the workflow is, the data and reporting it needs, and where it is hosted. Tell us who it is for, what it must do and any deadline, and Discovery turns that into a scope we can estimate from.

An example form or screenshot helps. Please use test data and never send patient information.
