(01)Challenge

Getting a patient on long-term ventilation home involves many teams and many steps. Delays build up when nobody can see what each discharge is waiting on.

(02)What we built

A secure web platform that takes a patient from referral to discharge: a structured referral form, real-time status tracking, a pathway tracker with traffic-light warnings, and a document bank for notes and letters.

It runs on the NHS Health and Social Care Network, so it can only be reached from NHS-connected devices. New users register in about five minutes; their NHS email is validated, an administrator approves each account, and access is limited to patients at the user’s own hospital.

(03)Result

Clinical teams can see where every discharge stands, which steps are running late and what needs attention next, and produce service activity reports with a single click.

What it does

  • Fast referrals

A structured referral form that cannot be sent without the essential information, with an email confirmation once it is.

  • Traffic-light warnings

Each step of the discharge pathway is timed against agreed targets and flagged red, amber or green.

  • Status in seconds

Designed so a patient’s status can be updated in around 30 seconds.

  • Document bank

Generate patient-specific notes and letters from data already in the system, and export them to PDF for the medical record.

  • Service overview

A live view of service activity, with automatic warnings when key measures near agreed thresholds.

  • One-click reports

Pre-built activity reports for current and discharged patients, with search and export built in.

Screens use test data, with names, NHS numbers and NHS logos removed. Platforms commissioned by NHS organisations are owned and operated by those organisations.
