Recreated e-VENT events screen for a demonstration referral: the referral table, the to-do list (add new, insert event details, inform related professionals) and two events, a housing needs assessment and team recruitment, each with subject, date, time and location. Patient identifiers are cleared.

(01)Challenge

Discharging a child on long-term ventilation involves many agencies, and organising the right care at home could take months.

(02)What we built

A secure web-based pathway shared by hospital, community and social care teams.

Key actions are tracked against agreed timeframes, with automatic email alerts when they slip, plus equipment lists and templates for professional reports and discharge summaries.

(03)Result

e-VENT won an NHS Innovation Award for Healthcare ICT in 2009. Royal Brompton named a key factor: a clinical specialist and a developer built it side by side. Read the press release (archived copy).

The e-VENT screen is recreated from the archived application with a demonstration referral and patient identifiers removed. Platforms commissioned by NHS organisations are owned and operated by those organisations.
