APACHE II Checker.
Every step, explained.
Explore the standard adult critical-care point score, with physiology, age and chronic health explained.
Build the point score
Use the value giving the most points during the first 24 hours. This standard score is distinct from the ICNARC prediction model.
Circulation, breathing and temperature
At FiO₂ below 50%, enter arterial PaO₂. At 50% or above, enter the A–a oxygen gradient instead, from a paired assessment.
Laboratory results
Measured haematocrit is used directly. Bicarbonate replaces pH; the two are never counted together. Changing oxygenation or acid–base type clears that input.
Age, neurology and chronic health
Use an assessable GCS, following the scoring definitions. With qualifying chronic disease, elective postoperative admissions receive 2 points; non-operative and emergency postoperative admissions receive 5.

Educational score checker for adult critical care. The point score alone does not provide a validated individual mortality percentage. This is not the ICNARC v4 mortality prediction model.
Standard APACHE II point score using the worst-scoring first-24-hour measurements. This is distinct from the ICNARC prediction model. No individual mortality percentage is inferred.
What this APACHE II calculator shows
The checker adds the acute physiology, age and chronic health components of the adult APACHE II score. Its breakdown shows the points assigned to each entered value, so the total can be inspected rather than treated as a black box.
Using the point breakdown
Follow the labels for units, oxygenation and acid-base inputs. The checker identifies missing or invalid inputs instead of presenting an incomplete total as a finished score. Use the sample scenarios to explore the scoring rules.
Scope of the result
This educational checker calculates a point score, not a personalised mortality prediction. It does not replace clinical judgement or independently validated clinical software. Calculations run in your browser; do not enter patient identifiers.
